Safety Warning

- Always maintain communication with attendant outside the confined space
- Exit immediately if hazardous conditions develop or alarms sound
- Never attempt rescue without proper training and equipment

Hazard Assessment and Planning
Identify hazards and develop entry plan

Tools: Hazard assessment form, Confined space inventory, P&IDs, Safety standards
- Identify all potential hazards associated with the confined space
- Develop specific entry procedures based on hazard assessment
- Determine required PPE, monitoring, and rescue equipment
- Assign roles: entrant(s), attendant, entry supervisor

Equipment Preparation and Isolation
Prepare the confined space for safe entry
2-4 hours
Tools: Lockout/tagout equipment, Blind flanges, Ventilation equipment, Gas testing equipment
- Isolate the space from all energy sources (LOTO)
- Isolate from all process connections using blinds or double block and bleed
- Drain, clean, and purge the space of hazardous substances
- Verify isolation is complete and effective

Atmospheric Testing
Test atmosphere to ensure safe entry conditions
20-30 minutes
Tools: Multi-gas detector, Calibration equipment, Sampling equipment, Documentation forms
- Calibrate all monitoring equipment before use
- Test atmosphere for oxygen content (19.5-23.5%)
- Test for flammable gases (<10% LEL)
- Test for toxic gases (H₂S, CO, etc.) below permissible exposure limits

Permit Issuance
Complete and authorize entry permit
15-20 minutes
Tools: Entry permit form, Atmospheric test results, Isolation verification, PPE checklist
- Complete all sections of confined space entry permit
- Review permit with all entry team members
- Obtain necessary signatures and authorizations
- Post permit at entry point

Entry Operations
Perform safe entry and work activities

Tools: Communication devices, PPE, Continuous monitoring equipment, Entry log
- Establish communication system between entrants and attendant
- Don all required PPE and check operation
- Set up continuous atmospheric monitoring
- Maintain attendant at entry point at all times

Emergency Response Preparation
Prepare for potential emergencies
30 minutes
Tools: Rescue equipment, Emergency communication devices, First aid kit, Extraction harness and tripod
- Position rescue equipment near entry point
- Review emergency procedures with all team members
- Ensure rescue team is available and notified
- Verify emergency communication systems

Completion and Closure
Safely complete work and close permit
30-45 minutes
Tools: Entry permit, Equipment inventory, Closure checklist, Documentation forms
- Ensure all personnel and equipment are out of the confined space
- Account for all tools and equipment used inside the space
- Close and sign off on the entry permit
- Return equipment to normal operating condition if appropriate
